Phase 5: Implementation of Aspirational Pathways
- Evaluate the impact of Pathways on children, families, practitioners, organizations, and communities through focus groups and surveys at set intervals.
- Supervisors/organization leaders to complete the IEMH Core Components Evaluation Tool (Post-Survey).
- IEMHP to share summary report of surveys, focus groups, and IEMH Core Components Evaluation tool (pre & post implementation).

Phase 1: Community Engagement
- Identify key partners from various sectors.
- Onboard community partners onto the IEMH Care Pathways initiative.
Phase 2: Service Mapping and Inventory Assessment
- Assess the current capacity, knowledge, and skills of community services.
- Map out current processes in the community, identify strengths and challenges.
Phase 3: Building the Aspirational Pathways
- Supervisors/organization leaders to complete the IEMH Core Components Evaluation Tool (Pre-Survey).
- Share report of the IEMH Core Components Evaluation Tool.
- Develop the Aspirational IEMH Care Pathways.
- Confirm Local Pathways Implementation Team and Central Hub agencies.
- Meet with Central Hub agencies to review ASQ Database documents and begin the approval process.
- Develop Family Friendly Resources.
- Conduct focus groups with families to review the family friendly materials.
- Develop and finalize Supplementary Document with the Local Pathways Implementation Team/Working Group.
- Develop final drafts of dissemination materials.
- Community develops a communication and dissemination plan.
Phase 4: ASQ and DSP Training and Implementation
- Complete ASQ/DSP training for Stream 1 and Stream 2.
- Determine schedule for ASQ data collection and community reports.
- Implement ASQ data collection process.
- Analyze results from Pre-Training Surveys and Pre-Pathways Implementation Survey.
- Evaluate current system (or services) using Pre-Pathway Implementation Survey and Focus groups with Leadership/Supervisors/Managers, Frontline Practitioners, and Families.
- Share summary report of surveys and focus groups.
Phase 5: Implementation of Aspirational Pathways
- Disseminate and implement IEMH Care Pathways in the community.
- Collaborate with partners to ensure they develop a sustainability plan for their community.
